Raycey McDonald - “Summing up the evidence to the jury, assistant coroner Darren Stewart said there were opportunities to keep a closer eye on Mr McDonald in the lead-up to his death on November 5, 2020. In particular, the failure to act on him reporting to staff he had heard voices telling him to take his own life. His family also told the inquest he made paranoid statements about being killed by fellow prisoners and guards.”
